SASMI’s Annual Physical Benefit

As of January 1, 2021 SASMI has added a benefit to support and encourage annual physicals / wellness checks. The benefit provides an incentive of $200 for the member and $100 per dependent who is active in the Local 265 Welfare Fund. The family maximum value is $600. You can review SASMI’s website and its Summary Plan Description for additional information and eligibility rules.

To apply for the benefit, you need to log into SASMI. Each member can log into SASMI’s portal found here: https://www.sasmi.org/member-portal-login/

Once you have logged into the site you will need to select the Applications tab at the top of the screen.

Under the Online Applications you will see the Annual Physical link.

Select the link and the Annual Physical Application will be shown.

You will then be asked:

• Who had the physical exam
• Relationship of the person examined
• Name of person examined
• Birthdate of person examined
• Facility Examined at
• Examined by (Name)
• Date of Exam
• Type of Examination: (Examples: Annual Physical, Female Wellness Checkup)
• You will need a copy of your annual physicals EOB (Explanation of Benefits) to upload into the site.
• You will need to upload an image of your insurance card.

All fields must be completed to successfully receive the incentive. Once completed, select Apply.

Your payment will be sent via ACH directly to your Bank. You will need your bank routing information and a voided check to upload.

The application is quite simple and ensures a safe and quick turnaround.
